Interchange is the most experienced Community Inclusion specialist
provider in WA and we are proud to have been providing customised,
person-centred support for more than 25 years.
We offer customised employment exploration and transition services, which
are designed to support people to discover their own unique skills and talents,
and harness those talents to identify and create a job role or small business
that is tailored specifically to them.
In addition to supporting our customers to become actively involved and
included in their community, our Community Facilitators can assist customers to
develop the skills they need to be able to live independently in their homes.
At Interchange, we recognise that some people prefer to manage their own
funding. As a customer or supporting family member, you can partner with
Interchange to share the management of your funding and services, giving you
greater control over when, where and how your funding is used. All the options are presented in a Shared
Management agreement. You can choose to do a little or a lot.
