What is the Bonaire Health Declaration form?
The Biomedical Admission policy (BSAP), which is a legally binding contract between you and Barbados, stipulates that you will be allowed to receive medical care in any public hospital in Barbados that has been approved by the Government. The policy sets out the criteria that must be met by your doctor to successfully complete your medical examination and undergo necessary treatment. The policy does not require you to pay any money. Your only responsibility is to complete the application process and to meet any other financial obligations that may be outlined in the document.
To be admitted to any public hospital in Barbados you will need to complete and sign the Bonaire Health Declaration Form. This document can be found at a variety of locations on the island. This document can also be found online. After you have entered your name, birthdate, and social security number on Bonaire Health Declaration Form you will receive an acknowledgment that the information has been updated. If you plan on traveling outside Barbados, you will need a copy and a copy your passport.
